MODEL RELEASE AND PARTICIPANT CONSENT

By signing this Agreement, or by completing the Company's electronic Model Release form prior to participating in the photoshoot or video production, I grant Liza Sue Productions, the company, organization, or individual commissioning the photography and/or videography (“Client”), and their respective employees, contractors, successors, assigns, agents, representatives, marketing partners, and affiliates, the irrevocable, perpetual, worldwide right and permission to photograph, film, record, edit, reproduce, publish, display, distribute, and otherwise use my name, likeness, image, voice, appearance, and any media captured of me during the photoshoot.

I authorize the use of such photographs, videos, and recordings for lawful business purposes, including but not limited to marketing, advertising, promotional materials, social media, websites, recruiting, internal communications, public relations, editorial purposes, portfolios, presentations, and other commercial uses.

I acknowledge that these photographs and videos are being created as part of the Client’s business activities and that the Client may use the resulting materials in connection with its ongoing operations, branding, marketing, and communications.

I understand that this authorization is voluntary, that I will not receive compensation or royalties for the use of my likeness, and that I waive any right to inspect or approve the finished photographs, videos, or other materials created from the photoshoot.

This authorization is binding upon me and my heirs, legal representatives, successors, and assigns. I understand that this release applies to all content created or captured during the photoshoot and may not be revoked with respect to any photographs, videos, recordings, or other media already created, captured, published, distributed, or in use.

I release and forever discharge Liza Sue Productions, the Client, and their respective employees, contractors, agents, successors, assigns, representatives, and affiliates from any claims, demands, or liabilities arising out of or related to the creation, editing, reproduction, publication, distribution, or use of my likeness, including claims related to privacy, publicity rights, defamation, compensation, or approval rights.

CONTINUING AUTHORIZATION AFTER EMPLOYMENT OR ASSOCIATION

I understand that photographs, videos, and other media created during this photoshoot may continue to be used by the Client for ongoing business, marketing, advertising, promotional, recruiting, internal, and commercial purposes, even after my employment, contract, affiliation, or relationship with the Client has ended.

I acknowledge and agree that my authorization and release are not dependent upon my continued employment or association with the Client. If my relationship with the Client ends for any reason, the permissions granted in this release remain valid, and the Client may continue to use previously created photographs, videos, and media containing my likeness in accordance with this Agreement.

I waive any right to demand removal, discontinuation, additional compensation, or additional approval for content that was lawfully created and authorized under this release.

If I am signing on behalf of a minor, I certify that I am the parent or legal guardian of the minor and have full legal authority to execute this release on the minor’s behalf.
